Where can I find my Medical ID card online?

  • Use the Provider Finder tool to compare prices for over 1,700 medical services, treatments, and tests.

What support is available for urgent or late-night health concerns?

  • Call the 24/7 Nurseline at 800-581-0368. Registered nurses are available anytime to help assess symptoms and guide next steps.

Does BCBS offer telehealth services/virtual healthcare visits?

  • Yes. BCBS gives you and your covered dependents access to MDLIVE for non-emergency medical care. You can connect with a board-certified doctor anytime, 24/7.

How do I create an account using the MDLIVE website?

  • Go to mdlive.com/bcbstx and click “Activate Now”
  • Enter your member ID and other information, enter your email address, and create a password
  • Click submit. Your account is now created. You should receive a confirmation email confirming your account has been created.

How do I create my Ameritas dental account?

  • Visit ameritas.com/sign-in and click “Member Sign In” under Personal Accounts
  • First-time users select “Register Now” and provide your User ID and password

How do I find in-network dental providers?

  • Use the Find a Provider tool at ameritas.com
  • Or use the Ameritas Benefits app on the App Store or Google Play

How do I access my dental ID card?

  • Go to the Ameritas portal. Once signed in, go to the Dental section
  • Click “ID Card” then “View, Print or Save” to generate a PDF of your cards

How do I submit a dental claim or view coverage?

  • Providers typically submit claims directly
  • If needed, use the Ameritas claim form and authorize payment to the provider
  • Check coverage through the Benefit Guide or your Ameritas portal

How do I create my VSP Vision Care account?

  • Visit the VSP Vision Care website or download the VSP app

How do I access my Vision Member ID card?

  • Log into your account at vsp.com or via the VSP app
  • From the homepage, select “View Member ID Card”

How do I file a vision reimbursement claim?

Does Kodiak have hearing plan coverage?

  • Yes, if you are enrolled in Ameritas’ vision plan, hearing coverage is automatically included.

Are hearing aids covered under our plan?

  • Yes, hearing aids are covered at 50% coinsurance, with no deductible.

HSA

What is an HSA?

  • A tax-advantaged account for qualified medical expenses, available to HDHP enrollees.
  • Contributions, growth, and eligible withdrawals are tax-free.

How do I use my HSA funds?

  • Use your Fidelity debit card for eligible expenses (copays, prescriptions, dental, vision)
  • Or pay out of pocket and submit a reimbursement request via the Fidelity portal/app

How can I change my contribution amount?

  1. Log in at myworkday.com
  2. Go to Benefits and Pay → Benefits Elections
  3. Select Change HSA Contribution
  4. Enter new amount and effective date, then submit

How do I get a new debit card?

  • Call Fidelity at 1-800-835-5095

How can I transfer or roll over an existing HSA?

  • Transfer: direct move from old custodian to Fidelity
  • Rollover: receive funds and deposit within 60 days
  • Benefits: simplified tracking, reduced fees, investment options
  • For assistance, call 1-800-835-5095

Dependent Care FSA

What is a DCFSA?

  • A pre-tax account to pay for eligible child or adult dependent care while you work.

Who qualifies?

  • Children under 13
  • Adult dependents needing daily support

What expenses are covered?

  • Preschool, daycare, summer camps
  • Babysitters, nannies, au pairs
  • Adult day care or in-home assistance
  • Transportation and late-pickup fees

How do I enroll and use my account?

  1. Enroll during open enrollment or after a qualifying life event
  2. Choose your annual contribution amount
  3. Use the HealthEquity portal/app to track spending and submit claims

What do I need for reimbursement?

  • Receipts or invoices showing provider and dependent names, dates, and costs
  • Submit via HealthEquity.com/login or the mobile app

How do I access my 401(k)?

  • Visit NetBenefits.com or 401k.com
  • New users can click “Register as a new user” to create an account
  • Once logged in, explore personalized financial options from the homepage

How do I change my 401(k) contributions?

  • Log into Fidelity NetBenefits and select your KODIAK GAS SERVICES 401(k):5981W plan
  • Click the three dots next to the plan name, then choose “Contribution Amount”
  • Click “Begin change contributions” under “Change Contributions”
  • Enter your preferred pre-tax and/or Roth contribution percentages (0–100%, in 1% increments)
  • Click “Change Contribution Amount,” then review and submit your changes
  • Changes are typically reflected within 1–2 pay periods

How can I request a 401(k) loan or withdrawal?

  1. Call Fidelity at 800-835-5097 or log into Fidelity NetBenefits
  2. Select your KODIAK GAS SERVICES 401(k):5981W plan and click the three dots, then “Loans or Withdrawals”
  3. Under “Request a new loan or withdrawal,” choose “Explore your options”
  4. Select a loan or withdrawal type, enter amount and repayment terms, then click “Calculate”
  5. Review your payment schedule, interest rate and fees, and first/last payment dates
  6. Click “Begin Loan Request” or adjust terms by selecting “Model Another Loan”
  7. Confirm your address and delivery method, then complete the review and submit process

 

Note: Loans are repaid via after-tax payroll deductions with associated fees.

How do I roll over retirement funds into my Kodiak 401(k)?

  • Visit Fidelity NetBenefits or call 800-835-5097
  • Select your KODIAK GAS SERVICES 401(k):5981W plan and click the three dots, then choose “Rollovers”
  • Under “I want to:”, choose “Move money into my KODIAK GAS SERVICES retirement plan”
  • Answer whether you have a check from your former retirement account (“Yes,” “No,” or “It was sent to Fidelity”)
  • Click “Get Started” and follow the on-screen steps

Employee Stock Purchase Plan (ESPP)

How does Kodiak’s ESPP work?

  • Purchase company stock via after-tax payroll deductions at a 15% discount with lookback
  • Eligibility: All U.S. payroll employees (excluding temps/interns)
  • Enrollment windows:
    • May 1–15 (effective June 1–Nov 30)
    • Nov 1–15 (effective Dec 1–May 31)
  • Enroll via Fidelity NetBenefits; a Fidelity Account® will be created for your shares

The Family and Medical Leave Act provides up to 12 weeks of unpaid leave for qualifying family and medical reasons. Eligibility requires:

  • Work site has 50+ employees within a 75-mile radius

What is SHARP?

Support of Higher Academic Reimbursement Program

 

SHARP is a Kodiak employee benefit program designed to support professional growth and financial wellness through tuition reimbursement, student loan assistance, and professional development funding.

What are the different programs under SHARP?

  • Tuition Reimbursement: Up to $9,250 per calendar year for accredited programs including technical certifications, associate’s, bachelor’s, master’s, or doctoral degrees that enhance their current role or provide opportunities for career advancement.
  • Student Loan Reimbursement: Employees who have been previous students and graduated from a college, university, or technical school needing financial support with student loans. The reimbursement is $200 a month.
  • Professional Development Reimbursement: Employees seeking to enhance their careers through continuing education, certification courses, training, or by attending seminars and professional/industry conferences. Reimbursement will be provided as an expense in Spark.

Who is eligible for SHARP?

To qualify, you must:

  • Be a full-time, active employee
  • Have completed at least 90 days of employment
  • Be in good standing with the company

How do I apply?

  1. Login to kgs.okta.com and open Workday
  2. In the search bar, type “create request”
  3. Select “all” in the request type section and click “OK”
  4. Select the SHARP program that you would like to apply to (e.g., student loan reimbursement)
  5. Fill out the application and click “submit”
